Assessment of Antisperm Antibody

It is estimated that, about 5-10% of male infertility is caused by sperm autoimmunity. In 10-15% cases of unexplained infertility in women, circulating sperm antibodies are observed. Antisperm–Antibody development is mainly related to Testicular Trauma, Orchitis, Genitourinary Tract Infection, Spinal Cord Injury and Homosexuality. Antisperm – Antibody can be detected in serum, seminal plasma, on the surface of spermatozoa and in cervical mucus, other female reproductive tract fluids (tubal fluid, follicular fluid and uterine fluid) and serum.

IgG (Immunoglobulin-G) & IgA (Immunoglobulin-A) are the two major immunoglobulin classes of Antisperm-Antibody.

The diagnosis of Antisperm-Antibody mediated infertility lacks a pathognomonic clinical picture, however it should be suspected if,

  • The semen analysis shows sperm agglutination or clumping in the absence of any clinical infection.
  • History of testis injury or surgery with low sperm motility.
  • Increased round cells i.e. leucocytes (Repeated Genital Infection).
  • Poor Post Coital Test results
  • Sperm 'Shaking' observed on Sperm-Cervical Mucus contact test.
  • Unexplained infertility.
  • Genetic predisposition.

To detect Antisperm–Antibody, number of tests are available, but the preferred tests are MAR (Mixed Anti-Globulin Reaction) & IBT(Immuno Bead Test). However, advantages with MAR test are, it can be performed directly on fresh, untreated semen sample, quick results (obtained in few minutes), is simple and repeatable.

Safety & Environmental Precautions

  • Store the kit in appropriate conditions as per the instructions given on the package.
  • Do not use the kits beyond expiry date given on the package.
  • All the reagents are potentially hazardous and it is strictly advised to follow the MSDS instructions.
  • All the patients' samples should be treated as potentially infectious. Laboratory personnel are advised to wear protective gloves, face masks, laboratory coats etc while performing the tests.
  • Dispose off all the waste generated, during and after the tests, in an appropriate manner as per the governing compliance laws of the land.
